Understand Your Physical Health with Phzio

Get a clear picture of your current wellbeing with Phzio's Physical Health Insights. Through comprehensive testing and one-on-one support from a dedicated care team — including a nurse practitioner, athletic therapist and dietitian — you’ll receive a personalized blueprint for better health.

    What’s included in the program:

    • Comprehensive lab tests, a heart test (ECG) and a movement assessment;
    • A summary of your results explained by a nurse practitioner; and
    • One-on-one consults to create a customized care plan.

    Access Phzio through the Connected Care portal in the Blue Cross Mobile app or Member Services Site.

    If you have additional health coverage through a spouse or partner’s plan, you could be getting up to 100% of your eligible claims reimbursed.

    Simply submit your claim through your group plan first, then provide a copy of the processed claim to your individual plan to maximize your coverage.

     

    Submit eligible claims through the Member Services Site or the Blue Cross Mobile app.
